Medical Information & Consent

Please inform us by email to info@ukfilmschool.org.uk if;

You have any medical condition, illness, allergy or phobia that may impact your ability to participate in the course or that we should be aware of.

You have been in contact with any contagious or infectious diseases within the last 6 months or suffered from anything that may be or become contagious or infectious?

Have you had Covid 19 or have been in contact with someone who has Covid 19 within the last 14 days.

Please do not attend Film School if you are currently testing positive for Covid 19 or are suffering any symptoms of Covid 19 e.g. high temperature, a new continuing cough, sore throat, aching body.

Consent & Release Form

I grant to UK Film School (UKFS) their legal representatives or assignees, and those acting under their permission and upon their authority

Permission to use;

• any films, photographs and sound recordings made by UKFS for the film in which I feature
• or any literary, dramatic, musical or artistic work or film
• or sound recording created by me
• or any performance by me of any literary, dramatic, musical or artistic work
• or film or sound recording
• included in the film or in any and all media by any and all means throughout the world for the full period of copyright, including all renewals, revivals, reversions and extensions

All consents required pursuant to the Copyright, Designs and Patents Act 1988

I waive all rights, which I may have in respect of the film under the Copyright, Designs and Patents Act 1988

The School may assign or license this agreement to any third party

UK Film School & Creative Media Institute (CMSI) will use photographic, digital images, film, video and moving content of students only in line with its policies and in order to demonstrate or promote activities relating to their provision.
